Contraception review.

This form is for a repeat of your combined oral contraception, combined contraceptive patch, or progesterone only contraception. It is for registered patients only and NOT for requesting emergency contraception (the morning after pill). The answers will be reviewed by a doctor or nurse. You will receive a text message once the medication has been sent to your nominated pharmacy. This may take up to 5 working days. If we need any additional information then we will contact you by telephone or text message. Please note you will need an up to date blood pressure reading to order a pill or patch containing oestrogen. Many people have their own machines to measure blood pressure which are not expensive to purchase. Alternatively you can have the blood pressure checked at most local pharmacies or by booking an appointment with an HCA at Bournbrook Varsity. It is not a requirement to provide a blood pressure for progesterone only medication.
